Primary optic atrophy, right eye

This code describes the degeneration of the optic nerve fibers in the right eye that is not secondary to another identifiable ocular or neurological disease. It results in a reduction of visual acuity and/or visual field defects due to the primary loss of nerve tissue.

Use this code when documentation specifies primary optic atrophy affecting the right eye, without an underlying cause such as glaucoma, inflammation, or compression. Supporting documentation should include ophthalmological examination findings confirming optic nerve pallor and functional deficits attributed to the primary atrophy.

• Differentiate from secondary optic atrophy (H47.22-) where an underlying cause is identified.
• Ensure laterality is clearly documented and correctly assigned (right, left, bilateral, or unspecified).
• Do not confuse with optic nerve hypoplasia (Q14.8) or other congenital anomalies.
