This code signifies an acute inflammation of the stomach lining accompanied by evidence of active or recent hemorrhage. The bleeding can manifest as hematemesis, melena, or positive stool guaiac, and is directly attributable to the acute gastritis.

Apply this code when documentation clearly indicates acute gastritis as the cause of gastrointestinal bleeding. This includes scenarios where endoscopy confirms acute mucosal inflammation with erosions or ulcerations that are actively bleeding or have stigmata of recent hemorrhage.

• Ensure documentation specifies "acute" gastritis and a direct causal link to the bleeding.
• Do not use for chronic gastritis with bleeding; refer to K29.51 or K29.61.
• Code any associated anemia due to acute blood loss (D62) as a secondary diagnosis.
