This code signifies an accumulation of fluid in the lungs (pulmonary congestion) and/or a pooling of blood due to poor circulation (hypostasis), often a consequence of heart failure or prolonged immobility. It indicates impaired gas exchange and can lead to respiratory distress.

Use this code when documentation explicitly states "pulmonary congestion," "pulmonary hypostasis," or "passive congestion of lung." It is frequently seen in patients with decompensated heart failure, renal failure, or those who are bedridden for extended periods.

• Do not use this code if a more specific cause of pulmonary edema (e.g., acute pulmonary edema due to CHF) is documented.
• Always code the underlying cause of the congestion or hypostasis first, if known and documented.
• Distinguish from pulmonary edema (428.1, 518.4) unless the documentation specifically uses "congestion" or "hypostasis."
